Us TOO International hosted a very successful conference call on February 13th called Intimacy and Prostate Cancer. The response to this program was overwhelmingly positive. The complete transcript and audio replay are available on the website at ustoo . This program featured Dr. Bullock, recognized in the 2005 publication, Best Doctors in America. Dr. Bullock specializes in treatment of cancer of prostate, bladder and kidney, including impotence erectile dysfunction or ED ; issues and solutions. The program also featured two couples who have faced this challenges and found solutions. Our couples were Jerry & Jo Ann Hardy of the Detroit, Michigan area, and Jim and Maureen "Mo" Kiefert, of Olympia Washington. Not only were our couples exceptionally articulate in communicating their struggles and success, both Jo Ann Hardy and Jim Kiefert serve on the Us TOO's Board of Directors. While impotence is a common challenge with prostate cancer treatment, our panel acknowledged that it can be a highly emotional issue. Most importantly, the program focused on the fact that MANY solutions are available today. Some examples include: Oral medication Viagra ; Vacuum Pump Injections Implants Amidst this list of clinical options, however, there was a practical core message to this one-hour program. Those couples who are most successful in reclaiming intimacy are those who: can acknowledge their feelings about the loss or shift in their intimate relationship are able to have frank discussions with their doctor and each other can keep the lines of communication open, even when it is difficult are committed to finding a solution and remain hopeful.
Christine Lee, a dermatologist in Walnut Creek, Calif. Retin-A can improve skin texture and fade dark spots and freckles because it causes skin cells to turn over more rapidly. It shrinks dilated pores and improves cell turnover within the pores so they are less likely to clog and become blackheads and whiteheads. But what has earned Retin-A its long-held reputation is its ability to affect the retention of collagen. Collagen is what gives skin its structure, firmness and elasticity. Repeated sun exposure breaks down collagen and, with age, cells produce less and less collagen to repair the damage. Skin wrinkles, sags and loses fullness. Retin-A does double duty in helping to boost collagen. According to research at Michigan, it has the potential to stop photoaging before it starts. ''The retinoids prevent the rise of collagenase after UV exposure, '' Dr. Voorhees said. Collagenase is what breaks down collagen. But regular use of a retinoid product also increases the amount of new collagen formed, research has found, and that new collagen will last for years. Susan Hobbs, 56, of Royal Palm Beach, Fla., a retired firefighter, said she spends a lot of time outdoors, and has been using Retin-A for about 15 years. ''And I really don't have a lot of wrinkles, compared to other people my age, " she said. ''I think that using Retin-A has made a big difference, '' Ms. Hobbs said. ''I think if I didn't use it, the sun damage would have really taken its toll.'' The results are not just cosmetic. Dr. Voorhees said that retinoids have been used to treat precancerous skin cells. Studies show that after two years of use, those abnormal cells returned to normal. So if Retin-A is so effective and so well studied, why isn't everyone using it? Many dermatologists blame a lack of patient education. ''It's one of the most misunderstood drugs, '' said Dr. Kenneth Beer, a dermatologist in Palm Beach, Fla., and a clinical investigator for Allergan, the maker of prescription Avage and Tazorac. He is also an Allergan shareholder. People use Retin-A too much, use it too often, experience negative side effects and then give up on it too soon, doctors say. The problem with Retin-A is that it may actually make skin look worse -- with redness, flakiness and peeling -- for up to eight weeks. ''But by 24 weeks, patients will see dramatic, marked improvements, '' Dr. Lee said. Ms. Hobbs said when she first started using Retin A, she broke out with severe acne. ''I remember I called the doctor and told him I was going to stop using it because my face had never looked worse, '' she said. But her doctor advised her not to give up. ''And he was right, '' Ms. Hobbs said. ''In another couple of weeks, my skin cleared up.'' Manufacturers have come up with ways to make it more tolerable, less irritating and more effective. Brand-name prescription versions contain emollient moisturizers and have more-advanced delivery systems for getting the active ingredient into the skin. These additions are the main difference between generic Retin-A and the pricier name-brand versions a large tube of a name brand will run about 0 versus about for a generic ; . But in every case, patience and common sense are required. ''You need to take six weeks to ramp up very slowly, but people will glob it on every night from the start and then call their doctor in a panic that their skin is red and peeling, '' Dr. Beer said. Doctors generally recommend using no more than a pea-size dab for the entire face. They advise using the product once every three nights or once a week if skin feels irritated ; for a couple of weeks, then.
Dear Representative Senator Name ; : First paragraph for writing to Representatives: I'm writing to encourage you to support legislation requiring health care plans to cover infertility diagnosis and treatment. Bills introduced by Rob Andrews D-NJ ; , HR 969 and HR 1852, and more recently by Anthony Weiner D-NY ; . Rep. Weiner has introduced legislation requiring insurance coverage of infertility treatments including up to four IVF attempts ; by all group health plans that also cover obstetrical benefits. First paragraph for writing to Senators: I'm writing to encourage you to support legislation requiring health care plans to cover infertility diagnosis and treatment. As a constituent, I writing to ask that you introduce legislation to require health plans to cover the diagnosis and treatment of infertility S. 874 in the 107th Congress ; . As one who has faced obstacles in my attempts to build a much-wanted family, including a lack of insurance coverage, I know first-hand the impact that the disease can have on those experiencing it. There is a great need for this legislation, which will assist the more than 6 million women and men struggling with the disease of infertility. I have faced my own obstacles while attempting to build a family, including a lack of insurance coverage, and I understand first-hand the impact that the disease can have on those experiencing it. Although infertility is a medically recognized disease, many insurance companies do not provide coverage for treatment. The large financial burden of some infertility treatments combined with the emotional and physical consequences of experiencing infertility is overwhelming. My desire to build a family helps me to face these obstacles, but I also need your support. In 1998 The United States Supreme Court ruled that reproduction is a major life activity under the Americans with Disabilities Act. Thus, infertility, in which the ability to reproduce is impaired, has far reaching impact. The ruling supports the need for infertility insurance coverage. Those with infertility are unfairly discriminated against when denied coverage for this disease while most other diseases are covered. Well-managed coverage will not burden insurance companies. Studies show that coverage adds only a few dollars per year to an insurance premium. Infertile couples pay the same premiums as fertile couples, but are not able to access this needed care. Although many diseases and medical conditions are covered by insurance, the disease of infertility is often excluded. Thank you for your time and consideration. I hope that you will support efforts to provide greater coverage of infertility treatments. Such legislation will help to fulfill the dreams of thousands of couples. For more information about infertility and about the costs and benefits of insurance coverage, I encourage you to contact The American Fertility Association, at 888 ; 917-3777. Sincerely, Name Address.

PREPARED STATEMENT OF JEFFREY WALLACE AND DEBRA WALLACE My son, Alex Wallace, began with his skin breaking out during eighth grade. We treated it the usual way with over-the-counter cleansers and creams. His skin continued to worsen to the point where we needed to schedule an appointment with out family physician to see what we could do about it. Our physician, Dr. Cabe, prescribed Minocin 100 mg twice daily along with Retin-A treatment that proved to be completely ineffective. His skin, in fact, was getting progressively worse and we certainly gave it sufficient time to work. Alex was very distraught about the state of his skin, especially since he said he was the one going through this and that none of his classmates were experiencing any breakouts of their skin at this point in time. We knew we needed to see a dermatologist as soon as possible. Dr. Cabe's nurse recommended we see Dr. Sassmannshausen as her daughter had gone to school with him and she was extremely impressed by him. On 2 29 got the referral to see Dr. Sassmannshausen. Upon meeting him, we were very comfortable with him. He was professional, kind and compaasionate. He told us that not only could he clear up the acne for good, but also he actually gave us a timeline. This immediately got our attention, as we could not believe that anything would truly get rid of it and that our son would at best end up with scarring. Dr. Sassmannshausen was very enthusiastic about a drug called Accutane based on his experience and told us about it. He indicated that not a lot of dermatologists in our community would prescribe this drug because it was felt to be controversial, but his experience with it was very successful and that he could help our son. This really got our attention. We actually had some hope for our son. Although I have tried, I cannot remember specifically all that we talked about during our initial visit, but I do know that it was very clear to me for a number of reasons that this was very powerful drug and not to be taken lightly. Dr. Sassmannshausen informed us that the course of treatment would include monthly checkups at which time we would be given the scrip for the following month's dosage. He never gave us more than a 30-day scrip at a time. He also stated that Alex would need to have periodic blood tests. Last but not least, Alex would need to regularly take the medication and use recommended moisturizers, as this drug seemed to dry the patient from the inside out. Our family agreed to treatment. We felt confident in Dr. Sassmannshausen's ability and experience to treat Alex and again we were given hope. We proceeded with the treatment using a team approach. We had Dr. Sassmannshausen as the lead, answering our questions and monitoring the treatment. Alex had to do his part, which included following directions and informing us of how he felt and what changes he experience. Our job as his parents was to dispense his medication as well as support and encourage him during this really difficult time for him. I clearly remember being very aware that Accutane was a powerful drug and not to be taken lightly. The fact that it could completely clear up severe cases of acne when antibiotics were ineffective, the fact that there was and actual time frame for it to work, the fact that there were other dermatologists who were ``uncomfortable'' prescribing the drug, the fact that it clearly stated on the packaging that if pregnancy occurred there would be birth defects, the fact that our pharmacist made a point of consulting with us each time we filled the prescriptio--all of these things made it abundantly clear that we needed to b extremely cautious and acutely aware of what was going on with our son. Mindful of the above, we were excited and hopeful that Accutane was really going to work for Alex. His life was literally on hold during the time he had acne. It took a lot of courage for him to go to school. This was time when his body was changing, he was trying to find his place among his peers, his hormones were at full speed ahead and all he could see was his face. It was as if that was all he was--a face, and it was a face he didn't like and it caused him a great deal of pain. He was clearly a different person. He didn't smile or laugh very often; he became very reserved as if trying to be invisible. Of course, there were kids who made fun of him starting each morning on the bus. Every single day, I had heart-to-heart talks with him. We talked about what he had experienced each day. I was not satisfied to just ask him how his day went. I wanted to know if he felt angry, depressed, neutral, etc. He knew his home was his safe haven from the cares of the day and he was always happy to get home to his space and orlistat.
Lessons in dermatology research: protecting vulnerable research participants by T. Howard Stone, JD, LLM "The money was good and the money was easy." He first tried a deodorant test. He chose the one he thought had the least chance of harming him, and says it was funny watching other prisoners smell his armpits and look for signs of irritation. He was a bit uneasy that the underarm lotion was unlabeled, but the he received each week smothered his concern. He went on to test hand and body lotions and soon realized the program's full financial potential. "Three or four tests at a time could mean real easy money. Foot powder tests and deodorants would bring you 0 per month, and hand creams a buck a day. You could be making 0 to 0 a month." Prisoner interview, in Allen M. Hornblum's "Acres of Skin: Human Experimentation at Holmesburg Prison." New York, NY: Routledge; 1998: 6 Legacy of early dermatology research Allen Hornblum's book, "Acres of Skin, " accented by numerous personal interviews of experiments conducted from the 1950s to the 1970s at Philadelphia's Holmesburg Prison, is one of the few historical accounts of the extensive and dubious use of prisoners as subjects in dermatology studies of agents used in popular skin care products, some of which--such as Retin-A tretinoin ; --are in wide use today. The lessons learned from Hornblum's account should resonate any time dermatology research involves people who may be deemed vulnerable as research subjects. In 1976, profound concerns about prisoners taking part in human research studies-- including those testing new dermatology agents or products--were expressed by the National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research in its report, Research Involving Prisoners [1]. Some of the National Commission's concerns were based upon findings that money appeared to be a strong motivation for prisoners to take part in the studies. As the result of its deliberations and concerns, the National Commission, which had been charged by the U.S. Congress to study and make recommendations about the protection of human subjects not already subject to federal regulation, advised Congress and the Secretary of the U.S. Department of Health, Education and Welfare predecessor to HHS ; that research involving prisoners as subjects should be significantly restricted. These recommendations, adopted in federal regulations and still in effect today, essentially prohibit investigators from using prisoners in the types of dermatology.

Merozoites enter red blood cells and develop through the so-called ring, trophozoite, and schizont stages. The erythrocyte provides the parasite with a safe haven from the host's immune system, but presents certain logistical problems with regard to access to nutrients and disposal of waste products. Parasite growth is supported by the ingestion of host haemoglobin. During a 48 hour cycle the merozoites burst from the mature schizonts, releasing cell debris, which causes a febrile episode in the host. Within minutes, merozoites invade new red blood cells and the cycle continues. After several cycles, some.

Dryness unmet by all other treatments, " said Patricia Wexler, M.D., assistant clinical professor, Mount Sinai Medical School in New York. The treatment involves a series of tiny superficial injections to the problem area, including underarms, hands, feet, forehead, and back of neck. According to Dr. Wexler, the treatment is quite long lasting. Patients may experience relief from sweating up to nine months with these injections, she said. "Botulinum toxin helps break the vicious cycle of anxiety over sweating that causes more profuse sweating, " said Dr. Wexler. "Knowing you can successfully eliminate embarrassing sweating that inhibits social and professional interaction is very liberating." According to Harold Brody, M.D., a dermatologist in Atlanta, Ga., dermatologic surgical tumescent liposuction of the fat and sweat glands performed under local anesthesia as an outpatient procedure by your dermatologic surgeon ; is a wonderful addition to surgical treatments for sweating under the arms. The treatment was pioneered by dermatologic surgeons. "After the skin is swollen with dilute solution of local anesthesia, very small instruments called 'cannulas' are used to suck out the sweat glands in a bloodless fashion, " Dr. Brody said. "With almost no bruising afterwards, decreased sweating occurs, and botulinum toxin use may be reduced or unnecessary. Also, the patient may return to work the next day." The important thing to note, said Dr. Elgart, is that excessive sweating can be treated as "one less thing to worry about." Di Ruth Ann Grant.

The worst case scenario 7 ; are foreign language name chains containing syntactically important particles or content words with lower case first letters. As long as all words in the chain are capitalised, an approximate analysis can be obtained by assigning the PROP word class to all members of the chain allowing for a functional structure like in 4a ; . The examples in 7 ; , however, contain the particles 'n', 'of' and the apostrophed inflexion morpheme ''s' in lower case letters. The only easy solution to this problem is to enter the most frequent of those English ; particles into the Portuguese! ; lexicon. Thus, 'of' as well as Dutch 'van' and German 'von' ; is listed as PRP, and -'s as genitive PROP M F S. Thus, 7a ; gets a fair internal analysis32 Massachusetts NPHR Institute N of N Technology P ; , while 7c ; has to live with a proper noun reading for the 's-morpheme - which at least guarantees name chain continuity Michael NPHR 's N Friends N ; . Only 7b ; remains a total failure, the colloquial short form of the English co-ordinator not being listed in my Portuguese lexicon. Are there alternatives to the semi-heuristic solution to the name chain problem proposed above? A short glance at the telephone directory of any larger town may convince even the most optimistic linguist of the futility of comprehensive dictionary cover for the whole word field. However, the real problem are not all the names that are treated heuristically, but only the ones that can also be assigned some convincing and provera.

Several peptidases collectively referred to as angiotensinase. Most metabolites of angiotensin II are biologically inactive, but the initial product of aminopeptidase action, [des-Asp1]angiotensin II, retains considerable biologic activity. Actions of Angiotensin II Angiotensin II acts at several sites in the body, including vascular smooth muscle, adrenal cortex, kidney, and brain 3 ; . Through these actions, the renin-angiotensin system plays a key role in the regulation of fluid and electrolyte balance and arterial blood pressure Fig. 6 ; . Angiotensin receptors and signaling pathways. Angiotensin II receptors are located on the plasma membrane of target cells throughout the body. Two distinct receptor subtypes, termed AT1 and AT2, have been identified 3 ; . Most of the known actions of angiotensin II are mediated by the AT1-receptor subtype. This receptor, which belongs to the G proteincoupled receptor superfamily, is a peptide containing 360 amino acids that spans the cell membrane seven times. Binding of angiotensin II to AT1 receptors in vascular smooth muscle results in the phospholipase C-mediated generation of inositol trisphosphate [Ins 1, 4, 5 ; P3] and diacylglycerol DAG ; . Ins 1, 4, 5 ; P3 mobilizes calcium from endoplasmic reticulum, whereas calcium and DAG activate enzymes, including protein kinase C and calcium-calmodulin kinases, that catalyze phosphorylation of proteins. These events result in smooth muscle contraction within seconds. They also initiate slower responses to angiotensin, including vascular growth and ventricular hypertrophy. The function and signal transduction pathways for AT2 receptors are still being investigated. Blood pressure. Angiotensin II is a potent pressor agent, considerably more potent than norepinephrine. The pressor response to angiotensin II is rapid in onset 1015 seconds ; and sustained during long-term infusions of the peptide. A large component of the pressor response to intravenous angiotensin II is due to direct contraction of arteriolar smooth muscle. However, angiotensin II also increases blood pressure through actions on the brain and autonomic nervous system. In particular, it acts centrally to increase.

Benzoyl Peroxide rapidly improves both inflammatory and noninflammatory lesions by reducing the population of P. acnes, and facilitating a decrease in the free fatty acids that ultimately lead to retention hyperkeratosis, and microcomedo formation." Cosmetic Dermatology April 1999 Sulfur and Resorcinol: Older medications which work on the surface of the skin to heal existing blemishes by unclogging pores. Tretinoin Retin-A ; : Tretinoin normalizes the exfoliation of the follicular epithelium and eliminates the microorganisms that encourage inflammation. It also drains clogged pores, supports the healing of blemishes, and prevents new blemishes from forming. ".tretinoin may enhance penetration of other drugs, such as topical antibiotics and antimicrobials Benzoyl peroxide ; , by facilitating the unplugged follicle to become less anaerobic and more accessible. This inhibits the growth of P. acnes and minimizes the rupturing of comedones into surrounding tissue. As a result, tretinoin is a first line agent for both inflammatory and non-inflammatory acne." Cosmetic Dermatology April 1999. Peeling, redness, irritation, dryness, and increased sun sensitivity are all possible side effects of tretinoin. When applied at acne sites in small doses, plasma Vitamin A levels are not affected, which suggests that topical tretinoin is a safe choice for pregnant women Tretinoin Formulas: Tretinoin is available in an assortment of formulations such as cream, gel, and liquid. Choosing a formulation depends upon an individual's skin type, skin condition, and environment. There have been several new developments in the Tretinoin field which include the introduction of generic formulations and a new microsphere formulation. The microsphere formulation was developed to diminish irritation, characteristic of the traditional Tretinoin. Instead of releasing the tretinoin into the skin upon application, the formulation contains the Tretinoin in a microsponge which attaches to the hair follicle before releasing the drug. The microsphere formulation has been proven to reduce twice the number of inflammatory and noninflammatory lesions as the regular formulation. Improvement, including decreased irritation, is evident in as quickly as two weeks. The first generic version of Retin-A, Avita, was approved by the FDA in 1997. Both formulations are composed of the same active ingredient, but the delivery system varies. A study illustrated that the efficacy of the two were comparable, though there are some lingering reservations with generic substitutes. The primary concern arises from a patient starting with the original product, who then switches to the generic version. Differences in innovator and generic products may trigger an allergic reaction this is a concern with all products, not just Tretinoin ; . Switching products during the course of treatment may ultimately result in minimal progress or treatment failure. Isotretinoin: More commonly referred to as Accutane, this drug affects all four of the contributing factors to the manifestation of acne. Accutane prevents comedogenesis by increasing exfoliation. It reduces sebum production, which may result in dryness in the skin, mucous membranes, and eyes. It also discourages inflammation, a result of the reduced growth of P. acnes. Generally reserved for individuals with severe and or cystic acne, Accutane is also prescribed for patients who have not responded to other treatments. The average treatment period is four months, and.

Of written materials by manufacturers, but the rationales for and against off-label promotion of this sort are relevant for all off-label uses. FDA explains its different treatment for promotion by the manufacturer and promotion by others with a number of reasons, including maintaining the incentive for manufacturers to study drug uses and FDA's mission to keep standards of safety and efficacy high.21 But FDA policy has created an "odd contradiction, " in that it purposefully declines to dictate to doctors how to prescribe drugs already been deemed safe for a particular use.22 Open season on prescribing allows companies to bypass the slow FDA approval process for new uses but also allows them to avoid the costs of conducting additional studies for review. Although this system makes drugs available more quickly to patients for any use deemed appropriate by their doctors, it encourages manufacturers to seek the most easily approved use before flooding the market with their products.23 In practice, once an off-label use generates enough buzz to be known to the public, a manufacturer has little incentive to conduct additional studies to get official FDA approval. Even though they are not allowed to promote the unapproved use as freely as they might wish, companies usually do not bother seeking approval of the new use unless they believe the drug in its new incarnation will be a blockbuster seller.24 Instead, most drug companies rely on their relationship with doctors and on doctors' talking to each other to promote off-label uses.25 It seems that most of the blockbusters are not for serious medical issues. One example aside from Botox and Retin-A is the discovery of the side effect of a blood pressure drug called minoxidil. Nowadays, minoxidil is marketed as Rogaine, the ubiquitous balding treatment with different formulations for men and women, in both liquid and foam formulations, even with different scents.26 Pfizer, Rogaine's.
