SUMMARY During atrial extrastimulation, split His potentials, prolonged His-to-ventricular H, V, ; intervals and block distal to the His bundle deflection were observed in both preoperative and postoperative children with heart defects. His-Purkinje responses and refractoriness were identified in 31 of 40% ; pediatric patients 20 of 51 preoperative and 11 of 27 postoperative ; during atrial extrastimulation coupled to sinus and or paced cycle lengths. Split His potentials were found in 14 patients eight preoperative and six postoperative ; and His bundle relative refractory periods ranged from 250-490 msec. Prolonged H2V2 intervals were found in these and in an additional 16 patients 11 preoperative and five postoperative ; and the relative refractory period of the His-Purkinje system ranged from 230-500 msec. Block distal to the His deflection occurred in seven patients five preoperative and two postoperative ; and the effective refractory period ranged from 230-510 msec. Split His potentials, long H2V2 intervals and block distal to the His bundle deflection produced by atrial extrastimulation were found in preoperative as well as postoperative children. These responses probably represent functional electrophysiologic characteristics of the pediatric cardiac conduction system.

Although the most prevalent cancer in the United States today, skin cancer is also the most preventable. The majority of the more than 1 million annual U.S. cases of skin cancer are sun-related. "The skin is the largest organ in the body and should not be taken for granted. In states like Texas where sun exposure can be intense for many months, we can take an active role in preventing skin cancer, " said Dr. Meghana Bhandari, medical oncologist at Texas Oncology . "Melanoma can be deadly, but it is curable if diagnosed early and has not spread to other parts of the body." Nationally, melanoma, the most deadly of all skin cancers, will account for more than 60, 000 cases of skin cancer in 2008 and more than 8, 400 of the more than 11, 000 skin cancer deaths during that time. Texas ranks third in the nation for incidence of malignant melanoma, and one in three Texans will develop skin cancer in his or her life. "Prevention starts by knowing what the risk factors are and taking measures to protect against ultraviolet rays, " said Texas Oncology Bhandari. "In addition, because early detection leads to higher survival rates, you should be mindful of changes in your skin and have them examined by a physician." People with fair skin or who are outdoors frequently are at higher risk. For example, people who work outdoors are susceptible to high exposure to ultraviolet rays. Parents should also remember that even children need protection at a young age from the sun. However, everyone is at risk of developing skin cancer and should take precautions. Department of Biochemsitry and Biophysics, and CVRI, University of California, San Francisco, CA 94143 A nitroxide spin label analogue of adenosine triphosphate has been used to examine the orientation of nucleotides bound to myosin in glycerinated psoas muscle fibers. Spin label ATP SL-ATP ; exists as isomers with the nitroxide free radical moiety attached by an ester linkage to either the 2' or 3' position of the ribose group of ATP BBA 601: 34, 1980 ; . SL-ADP has a binding constant of 105 with myosin and -2x103 in muscle fibers. When muscle fibers are incubated in 100-200 uM SL-ADP, the resultant EPR spectrum is characterized by two spectral components. The major component gives rise to three narrow peaks which are characteristic of unbound spin label. The other component is highly oriented and we interpret this to represent SL-ADP bound to myosin. Analysis of the hyperfine anisotropy is consistent with these spin probes subtending an angle of .45 with respect to the fiber axis. The relative molecular geometry of SL-ADP and myosin is unknown. Preliminary experients have indicated the probe angle does not change when tension is applied to the muscle fibers. Tension induces some broadening which is tentatively assigned as a small increase in the width of the angular distribution of the probes. SL-ATP will induce relaxation and will support fiber contraction. Tension and velocity measurements indicate that SL-ATP is -60% as effective as ATP.
